Step-1 :
Inform the insurer

Contact the toll free number and inform the insurance company of the hospitalization. In case of emergency contact the insurer within 24 hours of hospitalization and 7 days prior in case of a planned hospitalization

Step-2 :
Get Admitted in the hospital

Walk in the network hospital and show your health card to the hospital. This has all the necessary details required by the hospital to get authorization from the insurer

Step-3 :
Pre Authorization form to be sent to the insurer

Hospital fills the pre-authorization form with all the necessary details, scans and sends it to the insurer along with the supporting documents

Step-4 :
Send the documents along with pre-authorization form

Please note that the below mentioned documents is not exhaustive and you may be required to send other documents as required by the insurers

  • 1 : Policy Copy/ Health Card
  • 2 : ID proof with address
  • 3 : Investigation reports

Step-5 :
Insurer authorizes the hospital to proceed with cashless

Insurance company contacts the hospital, gets necessary information or documents to ascertain the authorization. Post the satisfactory receipt of all the supporting documents and information, the insurance company authorizes the hospital to proceed with the cashless service.

Step-6 :
Discharge from hospital

At the time of discharge, the hospital sends the final hospital bill and discharge summary in case of any residual amount. After the final authorization from the insurance company, the patient can be discharged. Any deductibles or co-payments will have to be borne by the customer according to the terms and conditions of the policy.

Step-7 :
Payment

After insurer's approval, the payment is done to hospital directly.

Get the treatment done as prescribed

Avail the treatment as prescribed and collect all the original bills, payment receipts and diagnosis reports from the hospital

Prepare your claim

  • 1 : Claim form duly filled
  • 2 : Policy copy
  • 3 : Hospital main bill
  • 4 : Hospital bill payment receipt
  • 5 : Pharmacy bill
  • 6 : Hospital break-up bill
  • 7 : Discharge summary
  • 8 : ECG
  • 9 : Investigation reports
  • 10 : Operation Theatre notes
  • 11 : Doctor's request for investigation
  • 12 : Prescription
  • 13 : Cancelled cheque

Step-4 :
Insurance company scrutinizes the claim

Insurance company scrutinizes the claim documents, gets necessary information or documents to ascertain the authorization. Post the satisfactory receipt of all the supporting documents and information, the insurance company accepts/rejects the claim basis the policy coverages

Step-5 :
Payment

The payment is done through NEFT/Cheque/DD to your account
