Step-1 :
Intimation of claim

Intimate the claim to the insurer as soon as possible. The claim intimation should consist of few basic information such as policy number, name of the insured, date and cause of death, place of death, name and relationship of the claimant.

Step-2 :
Submit relevant documents

The claimant will be required to furnish the below mentioned documents along with the claim form to the insurer. The list of documents is exhaustive and insurer might ask for few other documents if required

  • 1 : Original Policy Document
  • 2 : Completely & Counter signed filled Claim Form
  • 3 : Nominee ID proof establishing relationship with Life Insured
  • 4 : Copy of death certificate issued by local authority
  • 5 : Address Proof of the Nominee
  • 6 : Medical Records (admission notes, discharge summary, test reports etc)
  • 7 : Last Medical Attendance Report ( LMA)
  • 8 : Post Mortem Report ,if post mortem done
  • 9 : First Information Report / Police Report if case filed with Police
  • 10 : Panchnama / Inquest Report if case filed with Police
  • 11 : Saving Bank Account Number
  • 12 : Newspaper cuttings
  • 13 : Contact number of Claimant

Step-3 :
Claim Settlement

After the satisfactory receipt of documents by the insurer, the insurance company scrutinizes the claim and accepts/ rejects the claim basis the policy terms and conditions.
